Directions to Our Office

Arizona's Vision Eye Care Center is conveniently located in Phoenix at 15215 S 48th St STE 180, Phoenix, AZ 85044. We're easily accessible from major highways and offer ample free parking for our patients.

From Downtown Phoenix:

Head south on I-10 E, take exit 155 toward AZ-143/Sky Harbor Airport, merge onto AZ-143 S, continue onto I-10 E, take exit 160 for Baseline Road, turn left onto E Baseline Rd, turn right onto S 48th St. Our office will be on your right.

From Scottsdale:

Take AZ-101 Loop S to Phoenix, take exit 51B-51C for Baseline Rd, turn right onto E Baseline Rd, turn right onto S 48th St. Our office is located in the building on your right.

From Tempe:

Head south on S Rural Rd toward E University Dr, turn right onto E Baseline Rd, turn left onto S 48th St. You'll find our office in the building on your right.

Parking Information:

We offer free parking in the lot adjacent to our building. Handicap-accessible parking spaces are available near the entrance. Our facility is fully ADA-compliant with wheelchair access throughout.

What to Bring to Your Appointment

• Your current eyeglasses and/or contact lenses (if applicable)
• A list of current medications you're taking
• Your vision insurance card and photo ID
• A list of any vision concerns or questions you'd like to discuss
• Sunglasses (if you'll be having your pupils dilated)

New Patient Information

If this is your first visit to Arizona's Vision Eye Care Center, we recommend arriving 10-15 minutes early to complete necessary paperwork. This gives us time to review your medical history and ensure we can start your appointment on time.

You can also call our office ahead of time at 480-706-3937 to request new patient forms by email, allowing you to complete them at home at your convenience.

Insurance and Payment

We accept most major vision insurance plans including VSP, EyeMed, and many others. We also accept Medicare and most medical insurance plans for medical eye exams. Our knowledgeable staff will verify your benefits before your appointment and help you understand your coverage.

For patients without insurance, we offer competitive self-pay pricing and can discuss payment options for eye care services and eyewear. We believe everyone deserves access to quality eye care, and we work with each patient to find affordable solutions.

Emergency Eye Care

If you're experiencing a sudden vision change, eye injury, severe eye pain, or other urgent eye condition, please call us immediately at 480-706-3937. We prioritize urgent cases and will do our best to see you the same day.

For after-hours emergencies, please leave a message and our on-call doctor will return your call as soon as possible. For severe injuries or medical emergencies, please call 911 or visit your nearest emergency room.
